What is the bug:
Teleporting with an ender rail disables angel ring flight mode.

confirmed in 1.0.2 and 1.0.3

Known Fix:
Workaround: /gamemode 0 will reenable it.
 
Teleportation tends to deactivate baubles and the traveler's gear. This might be similar. You should be able to pick it up and place it back in your inventory to reset it.
