did you try talking to him? of course you didn't complete the mission, CustomNPC's sees a death by a block or entity as a death by the enviroment, meaning it had no player to bond the death to.

and then comes the question of a century:

Was the auto spawner on exact copy with a wither skeleton in the safari net?

Must be a bug with Ender IO. have you tried it with the MFR auto-spawner? or with a normal spawner in the nether (using a diamond dolly from JABBA)?

You can't open the town map. You're supposed to just hand it in.

If you are using an MFR Auto spawner, it should be set to exact copy. is it from ender io put it in the nether. also since it's a kill quest, you need to kill them your self. just don't spam 3 messages when you need only 1 to state your problem.
