It looks like you have 3 magnum torches. That is probably your problem.

However if you die 2 times from falling, you will not be able to log back in due to being "hardcore". That is why I had to restart my server.

The same thing happened to me and I had to restart my server. I changed it so it would force me into creative and I could fly over to the starting area and then I set the /jspawn. I then logged out stopped the server changed the gamemode and restartd the game and relogged in and started to play.

Do I need to update the server to 1.0.6? Looks like I do, so how do I get the 1.0.6 server?

I found mine in rivers usually interlaced into the sand but only 1 or 2 blocks under the water. We found 7 stacks amongst 3 of us in 1 MC night.

I believe that you need at least 12 or 16 villagers to spawn a golem so give it some time. He will spawn in the middle based on the door locations. 40 doors or 48? I think it requires 48 to work with out bringing in all the villagers.

How many doors are in the farm and how many villagers are in the farm?

Are you "throwing" it in or placing it in the controller?
