try without the TF2 teleporter mod? It's giving a bunch of errors and says your trying to load in with an item that it doesn't recognize.

Do you have any extra client side mods installed? sometimes Ars Magica doesn't work well with Optifine.

First it showed all kinds of errors on you trying to connect, now it just shows you are missing EE3, Mystcraft and Ars Magica... so download the file in the original post and unzip it in your mindcrack folder. Added.

You need to download the zip I posted in the thread and unzip it into your mindcrack folder. it'll add Mystcraft, Ars Magica and EE3
