Minecraft keeps crashing

I frankly can't tell, but it looks like there's a possibility it's being caused by some buggy block. Note that I'm guessing here.
Can you tell us what modpack you're using and if anything seems to have caused the problem?

Did you try changing compatibility options in the game? Go to Desktop. Right click Minecraft file > Properties > Compatibility > Run this program in compatibility mode for > Select a version you would like to run in. Then the next thing you’ve to do is changing NVIDIA Control Panel settings. Verify the state of Pre-Rendered Frames feature. If it is turned on, do disable it as showed in this guide. Remember, you also need to have latest driver versions installed to prevent Minecraft crashes.

It looks like you have a corrupt chunk or player file possibly. Use MCEdit to delete chunks from your world (careful, if you do this where your base is ... poof ... permanently gone!) or remove your player.dat file and test. Make a copy of your player.dat before removing it, that way if the crash still happens, you can put it back and then you know it's most likely a corrupt chunk.
